Why is the document needed?

It indicates the names and date of the drugs administered, and the local reaction of the vaccinee. The certificate is an attachment to the medical card. A certificate may also be issued in person. This document has the same legal force as a vaccination card.

Vaccination is the only method of protecting the country’s population from dangerous infectious viral pathologies. There are mandatory vaccinations and those that are optional. All of them are displayed in the medical book.

A certificate of immunoprophylaxis performed is necessary in order to understand whether a person is protected from infection with severe infectious viral pathologies and whether he poses a threat to others. Vaccination appointments are made by doctors. The certificate is certified by the doctor’s signature and the seal and stamp of the medical institution. If you fill out the form yourself according to the sample, then such a document will be considered invalid.

What is a vaccination certificate?

The preventive vaccination card in form 063/u or vaccination sheet is an A5 form on four sheets.

The vaccination sheet is created in the maternity hospital, since the first vaccination takes place there, then the form is transferred to the clinic.

The first sheet of the certificate contains general information about the accounting document and the person for whom it is registered. The header contains codes: forms for OKUD and institutions for OKPO, the name of the medical institution, and there is also a link to the regulatory document.

Below – after the name of the document – ​​personal information about the child is filled out, which contains the following information:

  • date of registration;
  • the name of the child care institution (when the child starts attending kindergarten and then school);
  • last name, first name and patronymic of the child;
  • date of birth;
  • home address;
  • note about change of address.

Further, from the first sheet to the end, tabular forms are provided to record vaccinations for each disease.

The tables will reflect information relevant to this type of vaccination:

  • date of vaccination;
  • the disease against which the vaccine was given;
  • type of vaccination (vaccination/re-vaccination);
  • name of the drug;
  • dose;
  • drug series;
  • vaccination site;
  • result, local reaction.

Vaccinations against tuberculosis, polio, diphtheria, whooping cough, tetanus, measles, mumps and other infections are mandatory. All information about vaccinations is accompanied by the signature of a medical worker and the seal of a medical institution. The last page of the certificate contains a field for entering information about the child’s deregistration (reasons and date), and also contains instructions for filling out the form.

Where and how to get a measles vaccination certificate?

When is a certificate for measles required?

An official certificate of measles vaccination in Moscow and other cities is issued in the following cases:

  • Getting a job (passing a medical examination);
  • admission of a child to kindergarten, school and other educational institutions;
  • hospitalization, health resort or health camp;
  • travel abroad.

A measles vaccination certificate is a universal option to cover absenteeism, confirm the fact of vaccination, and also resolve some other issues. In order for the completed measles vaccine form to be valid, it must be stamped and signed by doctors. The stamp of the medical center is also affixed.

How is the measles vaccination indicated in the medical record?

Who needs a medical clearance certificate?

In many regions of Russia, a decision has been made on mandatory vaccination of the population against COVID-19 (Order of the Russian Ministry of Health dated December 9, 2020 No. 1307 n). First of all, doctors and service sector workers are required to get vaccinated. For ordinary residents, vaccination is still voluntary and compulsory. But those who have not passed it risk facing a number of restrictions in the future - a ban on visiting restaurants, cafes, sports complexes, etc.

Residents of Russia have little choice. What category of citizens are not vaccinated?

  • allergy to quail or chicken protein.

If SNILS has not been provided and its data is not entered into the register, the patient will not be able to receive a certificate.

Another reason may be the lack of a confirmed account on the State Services portal. You can confirm your account online - on the websites and mobile applications of a number of banks, using an electronic signature, or by personally visiting the site's user service centers.

The certificate data will also automatically become available to those users who have confirmed their account on government services after undergoing vaccination.

How to solve a problem?

If you have a confirmed record, as well as SNILS for the first vaccination, but there is no observation diary in your personal account on the site, you must contact a medical institution to double-check the data entered in the register.

By the way, confirmation of immunization can also be obtained in paper form - upon request, medical institutions issue a certificate of vaccination, which has the same force as a certificate.

In addition, the regional health committee sent letters to medical organizations about the need to enter SNILS data into the federal register, and if citizens apply, to double-check the data.
