What is a vaccination card
A vaccination card is a paper that contains information about all vaccines and biological tests given to a person. A document is created for each child and adult upon registration at the clinic. The second name of the card is a vaccination sheet. It is filled out according to the approved form.
A vaccination card may be required in the following cases:
- admission to a higher or secondary specialized educational institution;
- registration for work;
- travel abroad;
- completing residency;
- registration at a local clinic;
- hospital admission;
- registration for kindergarten;
- sending for sanatorium-resort treatment.
A vaccination card will not be issued in person. If a person needs information about the immunization performed, he is given a certificate or an extract from this document.

The unified form of the document was approved by Order of the Ministry of Health of Russia dated December 15, 2014 No. 834n, in Appendix No. 19. The standard was last adjusted in 2021, the changes were presented in Order of the Ministry of Health of Russia dated January 09, 2018 No. 2n.

The document is valid for only six months from the date of issue. If a citizen did not have time to provide medical certificate 086/u at the place of request, then he will have to undergo a medical examination again.
Who creates and enters information on the vaccination list
An immunization card is usually obtained in the maternity hospital during the first immunization of the baby.
Next, the data is transferred to the department of the local clinic, the treatment and preventive office of the kindergarten, school, higher or secondary specialized educational institution.
The document is stored together with other cards and medical certificates about the person’s health status.
The vaccination list is constantly updated with new information about routine and emergency vaccinations. It is completed by nurses and local doctors of clinics and treatment and prevention centers.
If mass vaccination is carried out in a medical institution, then other vaccination paper can be used. But in any case, the data is then transferred to the medical record.

The original vaccination cards are always available in clinics. The person is usually given an extract or a copy of such a document. The card is stored in the file cabinet of the medical institution. Such documents are laid out according to the month of the next vaccination. This allows doctors to keep a record of those people who are subject to immunization in a certain month. The vaccination card is also in the archives at the paramedic and midwifery station. Such a sheet is stored for 5 years.
Nurses and family doctors have access to the information. Extracts from the vaccination card can be submitted to work, school and kindergarten. Immunization data is entered in the following columns:
- date of vaccination;
- What diseases does the product protect against (diphtheria, hepatitis, whooping cough, measles, tuberculosis or tetanus);

- series and name of the drug;
- site of vaccine administration (internal or external);
- result of the procedure (positive or negative local reaction).
The document also contains a column in which information about medical exemption from vaccination is entered (date and reason for such a decision).
The basis for refusing vaccinations is health problems. Before vaccinating a child, the pediatrician conducts an examination to exclude contraindications. The card is certified by the seal of the medical institution in which the person is registered.

What is indicated in the certificate
The record card for preventive vaccinations is drawn up on the official form 063/у. Each new entry is made after vaccination. At the same time, the date, dosage, name of the vaccine, where the injection was given, are prescribed. Also, information about the body’s reaction (allergies, redness of the skin, fever, diarrhea, etc.) is entered in a separate field. For the Mantoux test, the diameter of the reaction is indicated. This document records information about vaccination against local infections, about optional vaccinations done at the insistence of parents or before visiting countries with unfavorable epidemiological conditions. There may be information about allergies or special reactions to certain drugs, if present.
Difference between registration form 63 and vaccination certificate
Form 63 is an immunization card that is kept in a medical institution. It is intended to control vaccination. Based on this document, a vaccination plan is drawn up: the nurse determines when and which person to invite for preventive vaccination. A vaccination certificate is an immunization passport that a person has at home. It is necessary when going on any trips, when it is necessary to exclude one or another disease. Every time a vaccination is carried out, data about the procedure is entered into both documents: the card and the certificate.

What kind of document is this?
Parents, especially at the beginning of the school year, have a question about what kind of certificate 063/U is. Its sample was approved by order of the USSR Ministry of Health No. 1030 dated October 4, 1980. This order was already declared invalid in 1988, but with the proviso that the forms approved by it continue to be valid until new forms are introduced. These have not yet been developed, since the existing ones successfully perform the functions assigned to them. They store data on all vaccinations and revaccinations carried out since the birth of a person.
The card is stored by the medical institution that serves the patient, that is, the clinic at the place of residence.
